Fill out and submit our new Electronic Online Model Release which will open in a new window. Every field must be filled in and a legal signature placed in the box provided. Write the signature slowly and use the whole box provided for best results. It may take a few tries to get it right. Use the clear button if needed to start over. Print yourself a copy of the completed document. You will need it later. If you are unable to use the online form you will need to print and snail mail this Model Release (will open in a new window). NOTE: THIS INFORMATION IS STRICTLY CONFIDENTIAL AND WILL NOT BE RELEASED. Everyone appearing in your photos must sign a model release giving us permission to use the photos they are in.
