Step Two Model ID(s) Just four easy steps: STEP ONE - Photographer Information STEP TWO - Model ID(s) STEP THREE - Model Release(s) STEP FOUR - Photos We need a copy of ID for each person appearing in your photos! Photocopy, photograph or scan the photo ID(s). A Drivers License, Passport, "Green Card" or State Issued Identification Card are the only ID's that can be accepted. ALL information on the ID including the photo must be clear, in focus and legible! We need to be able to read all the information especially the birth date. (Example: Click for enlarged view.) You can use this form to email your ID(s) or you can do it from our Contact Page. Name: Email: Note: Attach ID:   Do not edit the ID(s) in any way! We blacked out the personal information in the sample for the models privacy. Hint: If you choose to photograph the ID(s) see if your camera has a "macro" or "close-up" function. This is often represented by a flower on the focus settings. This will allow your camera to focus close up to the ID(s). If you choose to photocopy aka Zerox your ID(s) you will need to Snail Mail them to us. NOTE: THIS INFORMATION IS STRICTLY CONFIDENTIAL AND WILL NOT BE RELEASED.
